FQHCs

Make sure a referral sent is a patient seen.

Health centers refer out more than almost anyone, to a specialist network that’s thin, slow, and different for every plan. Calvient tracks outbound referrals until a result comes back and handles the intake, eligibility, and outreach work piling up around them.

02 How we help

Three things we can take off your plate

  1. 01

Outbound referrals, tracked to a result, not just to “sent.”

  2. 02

Reach patients in a way they actually answer

03 Scenario

One patient, start to finish

  1. January

A patient with type 2 diabetes is due for her retinal screening. Her provider puts in the referral at the end of the visit.

  2. The same afternoon

Calvient checks which ophthalmology practices are in network for her MCO this year and currently accepting new patients, and queues the referral to the two closest to her home address.

  3. Two days later

She gets a text with the appointment offer and the clinic address. She taps to confirm. If she hadn’t, a call would have gone out on day five.

  4. She doesn’t go

The specialty office reports a no-show. Calvient doesn’t close the referral. It reopens it, moves it to your coordinator’s queue, and says why. A person calls her, and the appointment is rescheduled.

  5. March

She goes. The report comes back by fax eleven days later with no cover sheet. Calvient reads it, recognizes it as a retinal exam result, matches it to the open referral from January, attaches it to her chart, and closes the loop.
